I/We hereby authorize and consent to our child’s participation in PK Kids outdoor activities in which our child may participate, and that physical injuries may occur to our child requiring emergency medical care and treatment. I/We assume the risk of injury to our child that may occur in an outdoor athletic activity and Will Not Hold Responsible In consideration of the acceptance of our child by PK Kids PlayHouse for Disabled & Disadvantaged Children, Inc.
